CHILDHOOD IMMUNIZATION

WHO IS ELIGIBLE:
Infants, children, adolescents; birth through 18 years of age.

SERVICES OFFERED:
Clinics provide vaccines to protect children against Diphtheria, Pertussis, Tetanus, Polio, Measles, Mumps, Rubella, Hepatitis A, Hepatitis B, HPV, Influenza, Meningococcal, Rotavirus, Strep. pneumoniae, and Varicella. Pneumococcal vaccine is available to high risk children.

WHO PAYS:
A low cost administration fee, on sliding fee schedule. Service is not denied for inability to pay. Medicaid and some insurances accepted. Credit cards accepted.

HOURS:

Main Office location
8:00 AM - 4:30 PM Monday through Friday, appointments preferred.
Late hours: 3rd Thursdays until 6:00 PM.

Teen Health Center
8:00 AM - 4:30 PM Monday through Friday, closed 12:00 PM to 1:00 PM for lunch, appointments preferred.
Late hours: 1st and 3rd Mondays 10:00 AM until 6:30 PM.
